Qualifying Conditions
To qualify for a medical cannabis card in Illinois, you must be a resident of the state and suffer from at least one of the following qualifying conditions: cancer, glaucoma, HIV/AIDS, hepatitis C, multiple sclerosis, Crohn’s disease, Alzheimer’s disease, cachexia/wasting syndrome, muscular dystrophy, nail patella syndrome, specified pain disorders, rheumatoid arthritis, or any other condition that a physician determines could be helped by the use of medical cannabis.
Qualifying Conditions for an Illinois Medical Cannabis Card
In order to qualify for an Illinois medical cannabis card, patients must be diagnosed with at least one qualifying condition as determined by a licensed physician. Qualifying conditions include but are not limited to:
-Cancer
-Glaucoma
– HIV/AIDS
-Crohn’s disease
– Fibromyalgia
-Muscular dystrophy
-Seizures, including but not limited to those characteristic of epilepsy
-Spinal cord disease, including but not limited to arachnoiditis, Tarlov cysts, hydromyelia, syringomyelia, Rett syndrome, cerebral palsy, and post-polio syndrome
-Suffering from a terminal illness with a life expectancy of less than six months
The Application Process
If you are a resident of the state of Illinois and you think that you could benefit from the use of medical cannabis, you will need to obtain a medical cannabis card. The first step in this process is to fill out an application. The application will require you to provide some personal information as well as some medical information.
How to Get an Illinois Medical Cannabis Card
To get an Illinois medical cannabis card, you first need to get a written certification from a licensed medical cannabis doctor. The doctor will need to confirm that you have a qualifying condition that can be treated with medical cannabis.
Once you have your doctor’s certification, you will need to register with the Illinois Medical Cannabis Pilot Program. You will need to provide proof of residency, a government-issued ID, and your doctor’s certification. There is a $100 fee to register.
If you are approved, you will receive a medical cannabis card that will allow you to purchase up to 2.5 ounces of cannabis from a licensed dispensary every 14 days.
Fees
As of January 1st, 2020, the Illinois Department of Public Health (IDPH) will begin accepting applications for medical cannabis cards. The application fee is $100, and there is an additional $25 fee for fingerprinting services. The IDPH will also require a written certification from a licensed physician.

Illinois residents who wish to obtain a medical cannabis card must first submit an application to the Illinois Department of Public Health (IDPH). The IDPH will review the application and, if approved, will issue a medical cannabis card. There is a non-refundable application fee of $100 ($50 for veterans, seniors (65+), and persons with disabilities) which must be paid at the time of submitting the application. If you are a person with a terminal illness, you are not required to pay the application fee. Once your application is approved, you will be required to pay an annual renewal fee of $250 ($50 for veterans, seniors (65+), and persons with disabilities).
Renewals
If you already have a medical cannabis card in Illinois, you will need to renew it every year. The renewal process is pretty simple. You will need to fill out an application, which you can get from the Illinois Department of Public Health website. You will also need to provide proof of residency, proof of identity, and a current photograph.

If you need to renew your Illinois Medical Cannabis Card, here’s what you’ll need to do:
1. first, you’ll need to log in to the Illinois Medical Cannabis Pilot Program website using your user name and password.
2. Once you’re logged in, click on the “My Profile” tab at the top of the page.
3. On the left-hand side of the page, under the “My Profile” heading, click on the “Renewals” link.
4. On the Renewals page, you’ll see a list of all of your current medical cannabis prescriptions that are up for renewal. To renew a prescription, simply click on the “Renew” button next to the respective prescription.
5. Once you’ve clicked on the “Renew” button, you’ll be taken to a page where you can enter your new prescription information. After you’ve entered all of the required information, simply click on the “Submit” button at the bottom of the page.
