Introduction
The state of Rhode Island has a medical cannabis program that allows qualifying patients to receive a medical cannabis card. To get a medical cannabis card in RI, patients must first get a written certification from a licensed physician. Patients must then submit an application to the Rhode Island Department of Health. The application must include the physician’s certification, as well as the patient’s proof of residency in Rhode Island. Once the application is approved, the patient will receive their medical cannabis card in the mail.
Eligibility
You must be a Rhode Island resident with a valid RI driver’s license or state ID. If you do not have a RI driver’s license or state ID, you must provide a photocopy of a passport, out of state driver’s license, or out of state ID. You must also be at least 18 years old.

The Application Process

The evaluation will last approximately 30 minutes, during which the medical provider will review your medical history and make a determination as to whether or not medical cannabis would be an appropriate treatment for your condition.
Obtain a written certification from your medical provider
You will need to obtain a written certification from your medical provider in order to apply for a medical cannabis card in Rhode Island. The certification must be on the licensed provider’s letterhead and must include the following information:
· The provider’s name, address, and telephone number;
· The date of the certification;
· The patient’s name, date of birth, and address;
· The diagnosis made by the licensed provider; and
· The length of time for which the patient is being treated by the licensed provider.
Once you have obtained your certification, you will need to fill out an application form which is available on the Rhode Island Department of Health website. After completing the form, you will need to mail it along with a copy of your driver’s license or state ID, and a check or money order for the application fee to the address provided on the website.
Complete the online application
To complete the online application, you will need:
-Your Rhode Island driver’s license or state ID
-A credit or debit card for the $50 application fee
-An email address
If you do not have a Rhode Island driver’s license or state ID, you can use a passport. The application will take about 10 minutes to complete. You will be asked to provide basic information about yourself, your medical condition, and your medical provider.
Submit the required documentation
